Forklift accidents in Irish workplaces are not random. They follow a pattern so consistent it would be funny if people did not end up in hospital because of it.

The Health and Safety Authority's incident data shows powered industrial trucks as a recurring source of serious injuries and fatalities year on year. Warehouses, construction yards, food production facilities, farms with telehandlers. The machine changes. The failures do not.

The 1.2-Tonne Crate Incident and What It Actually Reveals

A 1.2-tonne crate drops from a forklift. Someone is underneath or nearby. The outcome ranges from life-changing to fatal depending on where exactly they were standing.

Investigators arrive. They find the load was not properly secured before the lift. The driver had not completed a pre-shift check. There was no defined pedestrian exclusion zone around the operating area. The forklift itself had a hydraulic issue flagged two weeks earlier and never fixed.

Four failures. Every single one visible before the shift started.

This is not an outlier. The machinery is not the problem. The problem is what happens in the fifteen minutes before the first lift of the day.

Why the Pattern Keeps Repeating

Familiarity is the enemy here. A driver who has operated the same forklift for three years stops seeing it. The hydraulic drift that started six months ago gets normalised. The pedestrian walking through the loading bay without a hi-vis becomes unremarkable. The unsecured load gets moved "just across the yard."

Supervision compounds this. Managers who do not operate forklifts themselves often have no real sense of what a compliant pre-shift check looks like versus one that is being performed for show.

The 10-Minute Daily Checklist That Changes the Outcome

This is not a tick-box exercise. Each item exists because someone was hurt when it was missed.

Before the operator touches the controls:

1. Walk the machine (3 minutes)

Check tyres for damage and correct pressure. Look for hydraulic fluid on the ground under the mast. Test the horn. Check that forks are not cracked, bent, or worn at the heel. Confirm the seatbelt works and is not frayed. Look at the overhead guard. If it is bent or has a crack, the machine does not move.

2. Function test before load (2 minutes)

Start the engine and raise the mast fully. Watch for hydraulic drift: if the forks drop more than a few centimetres with no load applied, there is a problem. Test forward, reverse, and both directions of steering. Sound the horn. Check that lights work if the machine operates in low visibility.

3. Confirm the load and the path (3 minutes)

Before picking anything up: check the load weight against the capacity plate on the machine. Never assume. A 1,200 kg crate on a forklift rated for 1,000 kg at a 500 mm load centre is an incident waiting to happen.

Confirm the load is on the pallet correctly and is not overhanging in a way that will shift during travel. Know the route. Know where people are. If there are pedestrians in the area, they stop moving before the forklift does.

4. Check the exclusion zone (2 minutes)

No one stands within 3 metres of a forklift carrying a load. This is not a preference. Mark it on the floor with painted lines if you have not already. Brief every pedestrian, visitor, and contractor who enters the area. Once.

Load Securing Is Not Optional

Most dropped load incidents trace back to one of three things: the load shifted because it was poorly stacked, the forks were not fully engaged under the pallet, or the operator drove too fast over an uneven surface.

All three are preventable.

Forks should be fully inserted under the pallet, at maximum width for the load. The load should be tilted back slightly during travel, never carried high. Speed on site around loads should be walking pace at junctions and blind corners. If there is an uneven floor surface, report it. It is a maintenance issue, not a driving skill.

Stretch wrap on pallets is not decoration. If a load is wrapped, check that the wrap is intact and tight before lifting. If it is not, rewrap it. This takes two minutes and prevents a crush injury.

What Supervisors Need to Do Differently

The checklist above only works if it is actually done. That means supervisors need to physically observe pre-shift checks at least once a week, not review the paper form afterward.

Any defect reported on a pre-shift check should be signed off as repaired or the machine should be taken out of service. A forklift with a known hydraulic issue that continues operating because "it's probably grand" is an employer liability waiting to become a prosecution.

Incident data from the HSA consistently shows that where serious forklift injuries occur, there was often a near-miss in the weeks before that was not formally recorded or acted on. Near-miss reporting is not bureaucracy. It is the only early warning system that works.

The Turn

Ten minutes of structured checking will not solve every forklift risk on your site. You still need traffic management plans, trained and certified operators, and a maintenance schedule that actually gets followed. But the checklist is what happens every single day, before anything else moves. It is the thing that catches the hydraulic fault before the load is in the air. It is the thing that spots the cracked fork heel before it fails under 800 kg. The incidents that end up in HSA prosecution files almost always had a moment, earlier that morning, where someone could have stopped them.

That moment is ten minutes long, and it happens before the first lift of every shift.
